Pathogenic variants in PNPLA2 are the only known cause of neutral lipid storage disease with myopathy (NLSDM). Pathogenic variants in another gene, CDS, cause NLSD with ichthyosis, also known as Chanarin-Dorfman syndrome.

The PNPLA2 gene encodes adipose triglyceride lipase (ATGL), an enzyme which catalyzes catalyzes the initial step in triglyceride hydrolysis in lipid droplets.
